A completed enrollment packet, including a medical assessment from the child’s pediatrician is required in order to place a child in a Bell Center program. To download the enrollment packet, click on the links below. Please note that the Medical Assessment portion of the packet must be completed by your pediatrician.
Once the completed packet is received, the Program Director will contact the family to schedule a Family Centered Interview, as well as the child’s individual evaluation. When completed, a child can begin receiving early intervention services at The Bell Center.
